The Return of Dexamethasone vs Droperidol

We would like to update on some success to one of the Latent Error of the Week Challenges.

Recently a hospital purchased Droperidol in almost identical vials to dexamethasone. The reporter was initially disheartened when, after poining this out to the nurse in charge of theatre, they returned to the facility some weeks later to see the same vials.

Recently however the dexamethasone vials have been replaced with others more easily distinguishable from the Droperidol vials.

We’re chuffed to bits with this one. It’s a win in our book.

We’ll give it a score of 22/31 (random). See if you can beat it.
